Patient Portal For New & Existing Patients

 If you are a new patient and have not called our office already to make an appointment, please do so.
 

After making your appointment, you will be invited via email to register at our patient portal for the first time. The patient portal is used to complete the necessary registration forms prior to your scheduled appointment. The portal can also be used to submit refill requests and send messages to our office staff.
 

If you have not received an email from our office to create your portal account, please contact us at (440) 617-9114.
 

If you are unable to complete the required registration forms from your computer, you will have to come to the office one hour before your appointment time to complete the registration process at our office.

In preparation for your first visit with our office, please bring the following:


1.  If you have insurance, bring your cards. Please have the name and birthdate of the primary subscriber of the insurance.

 

2. If your insurance requires a copay it will be collected at the time of service.

 
3.  If you are taking medications, please bring a detailed list.

 
4.  If your insurance carrier requires prior authorization, please make sure it is done prior to your visit.
